The process to be considered for a grant from Share A Smile is very straight-forward. Simply fill out and sign both the Application and Acknowledgement of Criteria forms and fax or mail to the Share A Smile Organization.
Once received, it will be reviewed by the Board of Directors to determine eligibility based on your current circumstances. Cases are reviewed on a quarterly basis and approval is determined by the Board of Directors. All grants are approved at the sole discretion of the Share A Smile Board of Directors and can be revoked at any time.
Applications for the 2011 Calendar year will be accepted during the following time periods:
1. January - February
2. July - August
